Effects of occupational therapy on cognitive function in patients with Alzheimer’s disease

Objective To construct the intervention effect of occupational therapy-based return to family intervention program on improving cognitive function and quality of life in patients with Alzheimer’s disease. Methods Through literature analysis and expert meeting,the return to family intervention program was formed. 68 patients with Alzheimer’s disease in the department of geriatric psychiatry who were admitted for the first time from April 2021 to October 2021 in a tertiary specialized hospital were selected. According to the community where the patients lived,Liangxi district was the study group and Huishan District was the control group,with 34 cases in each group. The study group carried out return to family intervention on the basis of routine nursing,including online communication,offline family visit,nursing clinic and community group activities,for 6 months. The control group received routine psychiatric nursing. The scores of Mini-Mental State Examination,modified Barthel Index Scale and Quality of Life Scale for Alzheimer’s patients were compared before the intervention,3 months and 6 months after the intervention. Results At 3 months and 6 months after the intervention,the scores of Mini-Mental State Examination,modified Barthel Index Scale,and the total score of Quality of Life Scale for patients with Alzheimer’s disease in the study group were higher than those before the intervention and the control group,and the differences were statistically significant(P<0.05). The compliances of the study group were 64.52% and 90.32% at 3 months and 6 months after intervention,respectively. Conclusion Return to family intervention based on occupational therapy can improve cognitive function,daily living ability and quality of life in patients with Alzheimer’s disease.

Establishment of an evaluation index system for health education among nasopharyngeal carcinoma patients with radio-chemotherapy

Objective We aimed to establish an evaluation index system for health education among patients with nasopharyngeal carcinoma(NPC) who are undergoing radio-chemotherapy. As a theoretical basis,this system may be applied by the clinical nurses to carry out targeted health education and guidance among NPC patients undergoing radio-chemotherapy and to evaluate the effect of health education. Methods From March to July 2021,we constituted a structured multidisciplinary research team,including oncologists,nurse specialists,nutritionists,et al,based on the theory of Knowledge-Belief-Practice. Literature review,semi-structured interviews,as well as the Delphi method and Multi-dimensional Analytic Hierarchy Process,were used to establish a framework of the evaluation index system and set weighted value for each evaluation index. Results The response rates of the 2 rounds of expert consultations are 85.0% and 100%.The expert authority coefficients are 0.932 and 0.935,respectively.The total Kendall harmony coefficients of the 2 rounds of expert consultations are 0.235~0.326 and 0.302~0.419(P<0.01).As a result,we built up an evaluation index system of health education which comprised 3 first-level indexes,19 second-level indexes and 73 third-level indexes for NPC patients undergoing radio-chemotherapy. Conclusion The evaluation index system may provide evidence and a practice tool for clinical nurses in conducting a comprehensive and efficient evaluation of health education among NPC patients undergoing radio-chemotherapy.

Objective To explore the application effect of acupoint application therapy combined with seed embedding in ear acupoints at selected time in patients with constipation(Heat syndrome) after endoscopic retrograde cholangiopancreatography(ERCP). Methods A total of 90 patients with constipation(Heat syndrome) after ERCP in a tertiary A hospital of traditional Chinese medicine in Suzhou from July 2019 to June 2021 were selected as the research subjects by convenience sampling method. They were randomly divided into an experimental group(n=45) and a control group(n=45).The experimental group was provided acupoint application therapy combined with seed embedding in ear acupoints at selected time intervention on the basis of routine care,and the control group was given acupoint application therapy intervention on the basis of routine care. The TCM syndrome points,efficacy index and first defecation time after intervention were compared between the 2 groups. Results The TCM syndrome points of (2.41±1.23) in the experimental group was lower than that of (4.17±1.56) in the control group (P<0.001). The efficacy index of (97.44%) in the experimental group was higher than that of (41.46%) in the control group(P<0.05).The time of first defecation after intervention in the experimental group was(10.03±3.98) h,which was shorter than (22.51±6.36) h in the control group(P<0.001). Conclusion Compared with single usage of acupoint application therapy,acupoint application therapy combined with seed embedding in ear acupoints at selected time can effectively improve the constipation (Heat syndrome) after ERCP by decreasing the TCM syndrome points,enhancing the efficacy index and shortening the first defecation time.

Evidence summary for the prevention and management of port-related venous thromboembolism in cancer patients

Objective To search,evaluate and integrate the relevant evidence on the prevention and management of port-related venous thromboembolism(VTE) in cancer patients,and to provide references for clinical practice. Methods We searched CNKI,the BMJ Best Practice,UpToDate,Cochrane Library,JBI Database,CINAHL,Embase,PubMed,China Guideline Clearinghouse(CGC),Scottish Interhospital Guidelines Network(SIGN),National Guideline Clearinghouse(NGC),American Intravenous Nurses Society(INS),National Comprehensive Cancer Network(NCCN),American Society of Clinical Oncology(ASCO),European Society for Medical Oncology(ESMO),and Registered Nurses’ Association of Ontario (RNAO) for relevant guidelines,systematic reviews,evidence summaries,recommended practices,and expert consensuses. The retrieval period is from the inception of databases to March 10,2021. Results A total of 12 articles were enrolled,including 4 clinical practice guidelines,5 systematic reviews,1 evidence summary and 2 expert consensuses. Totally 30 pieces of best evidence were summarized from 6 aspects,namely VTE risk assessment,catheter placement and maintenance,screening and diagnosis,prevention,management and treatment and health education. Conclusion This study summarized the best evidence on the prevention and management of port-related VTE in cancer patients. Nurses and nursing managers can select and apply the evidence according to clinical situation and patients’ wishes,so as to form a scientific,evidence-based,cancer port-related VTE prevention and management scheme,and improve nursing quality.

Effect of sensorimotor training in elderly patients with schizophrenia

Objective To explore the intervention effect of sensorimotor training on self-efficacy and activity of daily living(ADL) of elderly patients with schizophrenia. Methods 60 patients were divided into 2 groups according to their admission number. Patients whose last admission number was even number were assigned to the experimental group(n=30),and the patients with odd number were assigned to the control group(n=30). The experimental group received 10-week and 30-time sensorimotor training including 4 training modules based on the routine training and health education,whereas the control group received routine training and health education. Mini-mental State Examination(MMSE) was used to assess cognitive level of patients;the self-efficacy of patients was evaluated by General Self-efficacy Scale(GSES);Nurses’ Observation Scale for Inpatient Evaluation(NOSIE) was adopted to evaluate the condition and behavioral disorder of patients;Modified Barthel Index(MBI) was used to assess the ability of daily living of patients. Those scales were all used before and after the intervention. Results 10 weeks after the intervention,the scores of GSES in the experimental group were higher than those in the control group,among which the scores of orientation,memory,attention and calculation were significantly higher than those before the intervention. The factor scores of social ability,social interest and personal tidiness in NOSIE were significantly higher than those in the control group,while the factor scores of irritability,psychotic manifestations,withdrawal and depression in NOSIE were significantly lower than those in the control group. Scores of MBI were significantly higher than those in the control group except for defecation factor(P<0.05). Conclusion The sensorimotor training can improve the level of self-efficacy and the ability of ADL in elderly patients with schizophrenia.

Best evidence summary for sexual health management in breast cancer patients

Objective To search,evaluate and integrate the relevant evidence for sexual health management in breast cancer patients,and to provide references for clinical practice. Methods We searched CNKI,CBM,UpToDate,PubMed,Cochrane Library,Joanna Briggs Institute,CINAHL,National Guideline Clearinghouse,National Comprehensive Cancer Network,American Society of Clinical Oncology,European Society for Medical Oncology for relevant clinical decisions,guidelines,systematic reviews,evidence summaries and expert consensuses. The retrieval period is from the establishment of databases to February 10,2022. Results A total of 17 articles were enrolled,including 4 clinical decisions,5 guidelines,4 expert consensuses and 4 systematic reviews. Totally 33 pieces of best evidence were summarized from 7 aspects,namely screening and evaluation,psychological intervention,instrumental and exercise intervention,non-hormone therapy,hormone therapy,related reproduction and training and follow-up. Conclusion This study summarized the best evidence for sexual health management in breast cancer patients. In clinical practice,nurses can personalize the application of evidence according to clinical situation and patients’ wishes to improve breast cancer patients’ sexual health.

Analysis on the current situation and influencing factors of the self-management positivity of patients in the recovery period after simultaneous radiotherapy for nasopharyngeal carcinoma

Objective To understand the current status and analyze the factors affecting the self-management motivation of patients recovering from nasopharyngeal carcinoma(NPC) after simultaneous radiotherapy. Methods From January to June 2021,223 patients who were followed up in the radiotherapy department of a tertiary A hospital in Nanning City during the recovery period after synchronous radiotherapy for NPC were selected as the study subjects. They were investigated using the General Information Questionnaire,Positivity Scale,Self-Regulation Fatigue Scale,and Disease Acceptance Scale,and the factors influencing the self-management positivity of patients during the recovery period after synchronous radiotherapy for NPC were analyzed using univariate analysis and multiple linear regression. Results The positivity score of patients in the recovery period after simultaneous radiotherapy for NPC was (59.78±13.58),which was at a moderate level. The results of multiple linear regression analysis showed that patient literacy,self-regulation fatigue,and disease acceptance were factors influencing the self-management positivity of patients in the recovery period after simultaneous radiotherapy for NPC(P<0.05). Conclusion There is an urgent need to focus on the current status of patient motivation during the recovery period after simultaneous radiotherapy for NPC. It is necessary to develop a systematic and effective intervention strategy for motivation during the recovery period,to take into account patients’ literacy,self-regulation fatigue,and disease acceptance in a comprehensive manner.

To summarize the nursing process of a patient with portal vein thrombosis secondary liver failure after liver transplantation who underwent a liver re-transplantation. Nursing points:Based on collaboration of multi-disciplinary teams,during the process of artificial liver support treatment,the accuracy of the dosage of anticoagulant drugs was ensured;coagulation indicators were observed closely,and bleeding prevention measures were implemented. The measures to prevent thrombosis during the perioperative period of re-transplantation were taken and liver blood flow changes were dynamically monitored. During the implementation of minimal immunosuppressive therapy,acute rejection and infection were strictly prevented;during the concurrent chylofistula,a low-fat diet and drainage tube care were carried out. The patient was discharged on foot on the 63rd day after admission.
